Tacos, Tortillas and Tortas Fuel Mexico’s Obesity Problem

Dr Arya Sharma recently spoke at the XII Congreso Internacional Avances en Medicina 2010 hosted by the Hospital Civil de Guadalajara.

There is indeed no denying that Mexico, as does everyone else in North America, has a substantial obesity problem.

As recently pointed out by Dudley Althaus, who writes for the Houston Chronicle:

Though still afflicted by large pockets of the malnourished poor, Mexico is fat and quickly getting fatter, its children gaining weight faster than anyone else in the country.

About 70 percent of Mexican adults are now overweight, according to government estimates, more than triple the number of three decades ago. Also, about a third of the country’s schoolchildren and teenagers are overweight, making Mexicans the second-heaviest people on the planet, gaining quickly on their American neighbors.

With more people living in urban areas, and some with more money, Mexicans are eating more and exercising less. Forgoing the beans, tortillas, fruit and vegetables their grandparents ate, people flock to U.S.-style fast food.

Also, calorie-laden Mexican dishes once reserved for special occasions have become routine fare. Anything fried; packaged pastries, soft drinks and candy are widely available.”

Not surprisingly, last month, Mexican President Felipe Calderon launched a national campaign against obesity (beating the US Obama initiative by a few weeks) and called on parents, teachers and the food industry to guide people to more healthful living.

Whether or not this appeal will indeed have any noticeable impact on the obesity epidemic remains to be seen.  From Dr Sharma’s conversations with fellow delegates, at least the interest and concerns amongst health professionals seems considerable – but so does their sense of helplessness.

Bariatric Rehabilitation: Call to Action

In the past years, the Canadian Obesity Network has hosted two think tanks on the importance of rehabilitation expertise in the prevention and management of severe obesity. The proceeding of the first bariatric rehabilitation workshop held in Edmonton, Alberta in 2008 was now published in the latest edition of Disability and Rehabilitation.

Participants at this think tank consisted of around 50 health professionals, administrators, researchers and even EMS professionals.

The main goal of this workshop was to identify gaps in research and professional training that need to be addressed in light of the burgeoning number of individuals with severe and extreme obesity in Canada.

According to this paper:

“Gaps in knowledge and research included the need for better understanding of the biopsychosocial causes and consequences of obesity and the role for novel and existing rehabilitation interventions to prevent and treat obesity. Research is needed to explore how existing rehabilitation interventions and technology impacts the patients with obesity, which also has physical and psychological impairments typically treated by rehabilitation practitioners.

Although the need for rehabilitation for persons with obesity is documented in the literature, the capacity in terms of human resources, funding for research and technology and the development of bariatric friendly, accessible environments is limited. Participants of this meeting reinforced the need to develop training programmes in the form of continuing education and the inclusion of topics in bariatrics in discipline specific training programmes.

Additionally, in order for patients, their families and healthcare professionals to have access to obesity prevention and treatment services, models of care and education that reach out to rural and marginalized communities must be used. These currently exist through forms of telehealth, webinars and online courses.”

Clearly a bariatric rehabilitation is a topic that deserves far more emphasis in the training of health professionals and an area where there is an urgent need for rehabilitation research regarding best practices in dealing with this rapidly growing vulnerable population.

Do Brains of Obese Individuals Respond Differently to Food?

Readers of these pages are by now probably quite familiar with the complexity of ingestive behavior and the importance of understanding brain function in relationship to food intake.

A study, published in this month’s issue of Obesity, illustrates how differences in brain function between obese and non-obese people can explain important differences in response to food.

In this study, Laura Martin and colleagues from the Kansas Medical Center used functional magnetic resonance imaging (fMRI) to examine changes in brain activity in obese and normal weight adults while they viewed food and nonfood images in pre- meal and post-meal states.

Both in the pre-meal and post-meal state, obese participants showed increased activation in the anterior cingulate cortex (ACC) and the medial pre-frontal cortex (MPFC), regions of the brain responsible for the reward response and impulsiveness, respectively.

In addition, activation of the ACC was associated with decreased levels of self-reported dis-inhibition while MPFC activation was associated with increased self-reported hunger amongst obese participants.

These findings clearly suggest that brain function associated with food motivation differs in obese and non-obese adults and may well explain the different susceptibilities to weight gain and variability in response to diet interventions.

Given the emerging science on brain plasticity, it is certainly of interest whether or not these differences in brain function are acquired or are indeed innate. Whatever the case, we need to understand and acknowledge that our brains respond differently to the same food stimuli which easily explain why some people may find it much harder to resist overeating in our current obesogenic environment than others.

As I have said before, the obesity epidemic is simply the natural response to our unnatural environment.

Childhood Obesity Kills Native Americans, says Dr. Sharma’s blog

Besides being an expert in Obesity, Dr. Sharma also keeps the netizens updated with his informative blog posts related to Obesity. In one of the blog posts he talks about how childhood obesity may affect mortality.

The blog post reads as, “Despite the wide-spread concern about the health impact of the childhood obesity epidemic, there is actually not much data that directly shows how this excess weight may affect mortality.

Such data is now available at least for native American kids, from a study by Paul Franks (National Institutes of Health, Phoenix, AZ) and colleagues, just published in the New England Journal of Medicine.

The researchers analysed data from a cohort of 4857 American Indian (Pima or Tohono O’odham Indian) children without diabetes (mean age, 11.3 years) born between 1945 and 1984.

During a median follow-up period of around 24 years, death rates from endogenous causes among children in the highest quartile of BMI were more than double those among children in the lowest BMI quartile.

Similarly, rates of death from endogenous causes among children in the highest quartile of glucose intolerance were 73% higher than those among children in the lowest quartile and childhood hypertension was significantly associated with a 60% increased risk of premature death from endogenous causes.

Thus, at least in native Americans, childhood obesity is a significant risk factor for premature death – certainly a warning for those who believe that early obesity is something you can simply grow out of.

Given the raging epidemic of childhood obesity amongst the First Nations, Inuit, and Métis populations in Canada, these data should certainly prompt decisive action to address obesity amongst its native peoples.”

Born in 1960 in Indore, 49 years old Dr. Shailedra Trivedi has been serving the people of Indore for more than 25 years. M.D. in Medicine from M.G.M. Medical College, Indore (M.P.) and D.M. (Cardiology) from L.P.S. Institute of Cardiology of G.S.V.M. Medical College, Kanpur (U.P.). Dr. Trivedi completed his fellowship program in Interventional Cardiology from Prince Charles Hospital, Brisbane, Australia in 2001.

He has been fulfilling his duties in providing specialized services in cardiology since 1994 when he started his career as Senior Resident, DNB Trainee at Institute of Cardiovascular Diseases, Madras Medical Mission (MMM), Chennai. His work included Routine clinical and diagnostic work up, invasive and non-invasive, of all cardiac patients & management of cardiac emergencies, post op care etc. Had hands on experience on Transthoracic Echocardiography (more than 3000 cases), TEE (more than 100 cases), TMT & Holter Analysis (>500 cases), Diagnostic cardiac catheterization & Coronary Angiography (>100 cases). From November 96, He shifted to Institute of Cardiology, G.S.V.M. Medical College, Kanpur as D.M(card) trainee as a Sr. Resident. Here He was responsible for detailed work up of out patients & in patients including ICU care, Active participation in academic activities of the department and Non-invasive & Invasive assessment of these patients. He performed more than 50 diagnostic angiograms & catheterisations and more than 15 PPI. In 1999 he joined Institute of Cardiovascular Diseases, Madras Medical Mission, Chennai as a Staff Cardiologist where he took the responsibility of taking total care of all acute emergencies, chronic cases, preoperative and post operative care independently. He handled Non-invasive and Invasive procedures (coronary angiogram 8-10 cases/week, paediatric cardiac catheterization 4-5 cases per week) and Coronary and non-coronary interventions 2-4 per week. In January 2001 he got a chance to complete his fellowship at The Prince Charles Hospital, Brisbane, Australia, where he worked as a trainee fellow with Prof JHN Bett. Who had an excellent hand on training in interventional work by performing >700 diagnostic procedures, > 400 interventions as first asstt/first operator. He also actively participated in the academic activities of dept. and patient care along with cardiology trainees. He returned to Indore in 2002 at CHL Apollo Hospital as Consultant Interventional Cardiologist. Here he worked as full time independent Consultant. He started the cathlab and interventional work in a 100 bed new hospital, Performed 545 Diagnostic procedures, 68 coronary interventions, 24 BMV and other misc. interventions over a period of nearly 18 months. Then He joined Bombay Hospital as Consultant & Interventional Cardiologist. Here he established another new unit in the city, a 160 bed facility with all state of art equipments. He performed nearly 350 diagnostic procedures and 90 interventional procedures over 10 month’s period. He was also involved in teaching the resident staff, nurses. In 2004 he once again joined CHL Apollo Hospital as Consultant & Interventional Cardiologist. Now he is working again with the same unit as full time consultant, performing 40 to 50 diagnostic procedures, 12 to 15 coronary interventions, 4-6 non coronary procedures every month.

Since 2002 he is involved in delivering lectures and organising scientific sessions in Cardiology for the Family physicians, Internists and various speciality groups to help improve medical care of cardiac patients in the city and adjoining districts of state. He also conducted awareness programmes in society. He is immediate past Secretary of the Cardiological Society of India, Indore branch, where regular clinical meetings, CME programmes, workshops etc as part of educational activities are organised.

As a teaching and training professional his contribution is reflected in his active involvement in various training activities on national and international platforms. He has taught many undergraduates and post graduates during M.D., D.N.B & D.M. course. At MMM he regularly took sessions for D.N.B. trainees, Physician Assistants and Nurses. He was actively involved in Echocardiography training program held several times every year participated by doctors from all over India and also from abroad at MMM. At present he undertook teaching sessions for nursing staff and residents of the CHL Apollo hospital to update them on recent advances in cardiology. He held faculty positions at “CIT 2006, 2007 and CIT 2008” in conjunction with TCT and Euro PCR” held at Beijing in March 2007 and Singapore Live 2006 and 2009” held at Suntec city Singapore on international platform and Transradial angioplasty course organised at Holyfamily Hospital, Mumbai in Oct 2005 and “Advanced cardiac therapeutics 2008 and 2009” held at Madras Medical Mission, Chennai on national platform.

American College of Healthcare Sciences announces that Moses Untulan, a 21-year-old Vancouver, WA resident and student in the ACHS Associate of Applied Sciences in Complementary Alternative Medicine degree program, is the winner of the ACHS 2009 Be Part of the Solution video contest. According to the agreement, Infonetica Ltd. will provide the NIHR CRN CC with 1,200 training licenses for the next six months, which shall be used to support and train NHS staff working on CRN portfolio studies.

The training is entirely online based and includes multiple sections, which are organized within a core individual account per every trainee. The sections include useful documentation, glossary, and a bookmarking utility, in addition to ICH-GCP course information itself. In addition, there is an administration panel, which allows for management and exportable data analysis of multiple trainee accounts. The training has been designed to be used as either main or auxiliary means of providing ICH-GCP training and certification. The 30-question multiple choice exam may be taken at any point of the training and on multiple occasions until successful completion. Therefore, it allows individuals to work at their own pace and administrators to receive instant information about the progress of trainees.

The certificate, which is obtained only after successfully answering 80% of the exam questions, is available as a downloadable .pdf file. This makes it easy to store digitally or print out, depending on personal needs over time. Furthermore, once obtained and downloaded, the certificate may be edited and re-downloaded to reflect changes in the individual’s personal or work details.

New ActivePure Technology proven to reduce exposure to Methicillin Resistant Staphylococcus Aureus (MRSA)

A new technology known as ActivePure (RCI), developed by Tennessee-based EcoQuest International, has been proven in peer-reviewed, published university laboratory studies to reduce Methicillin Resistant Staphylococcus aureus (MRSA) on inoculated surfaces.

Recent outbreaks of MRSA in many school districts have had school administrators and parents concerned.

Based on NASA research, ActivePure (RCI) has been proven in laboratory testing at Kansas State University to reduce MRSA by more than 99 percent in 24 hours on surface areas. This has enormous implications for reducing exposure to infection-causing germs.

“ActivePure safely inactivates germs in the environment 24 hours a day, without sprays or wipes, the same way it happens in nature,” said EcoQuest Founder Mike Jackson.

“The use of ActivePure can reduce exposure by inactivating a majority of harmful bacteria, viruses and mold from surfaces in the protected area,” said Dr. James Marsden, Regents Distinguished Professor at Kansas State University and a lead researcher involved with the testing.

“In combination with good hygiene practices, such as hand washing, a reduction in bacteria and viruses on environmental surfaces could reduce the risk of infections,” said Marsden.
